Tracy Swanson

As an RN Client Services Coordinator Tracy helps their clients navigate the intake process, coordinates care, has ongoing communication with the client and their family, and provides referrals for services that Cypress does not supply.

Tracy is a Registered Nurse with over 30 years of experience in medical surgical, skilled care, women’s health, labor and delivery, hospice, elder law care coordination, and care management. She continues her passion for the elderly in assisting with care navigation and advocacy. When she is not working she and her husband enjoy spending time with their labradoodle, their grown children, and especially their granddaughters.
