Alfred Hammonds, Jr.

Al Hammonds, Jr. is the Executive Director, Millennium Collaborative Care. Among his responsibilities, he leads and coordinates projects toward successful completion of DSRIP project metrics and milestones; organizes and implements clinical redesign initiatives related to DSRIP projects in collaboration with health care providers and community based organizations throughout the eight-county region of WNY; compiles, analyzes, evaluates, and review operational documents such as program reports, data, and financial reports; and ensures compliance with DSRIP project rules and regulations.

With over three decades of administrative/management experience, Al is also a Certified Six Sigma Black Belt. He has been recognized with the UB Service Award at the 2008 Graduate School Awareness Conference, numerous American Society of Quality Awards (2008-2010), an a Black Western NY feature article in 2015.
