Sara Carpenter

Sara Carpenter, LMSW C-ASWCM, currently serves as a Medical Case Manager at Excellus BCBS, where responsibilities include conducting case management activities aligned with various health standards and collaborating with providers and community resources. With extensive experience in case management across multiple organizations, Sara has successfully managed care for Safety Net members, developed person-centered service plans, and improved departmental efficiencies. Previous roles include positions as a Social Worker in skilled nursing facilities, a Medical Case Manager at iCircle Services, and various internships related to social work.
