Autumn Wade

Director of Outpatient & Long Term Care Services at Flagship Rehabilitation

Autumn Wade has been with Flagship Rehabilitation since 2001. A proven clinical operations leader in various healthcare settings, Autumn started her career as an occupational therapist and was subsequently promoted to various operational roles. Currently, as Director of Outpatient/Long Term Care Services, Autumn is responsible for the clinical operations of Outpatient and Medicare B services for Flagship Rehabilitation.

Autumn attended Penn State University, Pennsylvania, where she obtained a Bachelor of Science degree in Occupational Therapy. She has completed numerous clinical and operational continuing education classes and continues to practice as an occupational therapist. Autumn is instrumental in the development of educational programs for Flagship Rehabilitation and provides expert insight through her certifications; Certified Lymphedema Therapist, Certified Stroke Rehabilitation Specialist, and Certified Dementia Practitioner.

Autumn’s leadership promotes engagement and strengthens partnerships through evidence-based practice to achieve outcomes at every level of the continuum.
