The Community Alternatives Program for Children (CAP/C) provides cost-effective services for medically fragile children (through age 20) who would otherwise require long-term hospital care or nursing facility care. Footprints provides quality case management services that allow the families to connect to resources in their community that  contribute to the quality of life for the children and their families/caregivers, that is cost-effective in comparison to the Medicaid cost for institutional care.  Owned and operated by a parent of three children with special needs, and many of our staff has children with special needs as well, we understand the unique challenges you face as the parents of a child with special needs

The current leadership team at Footprints Case Management has a wide range of experience in the field of nursing. The team includes an RN Case Manager who previously worked at Woody's Mom Inc. and a Pediatric Case Manager who previously worked at BAYADA Home Health Care. Both leaders have a wealth of experience in nursing and have been educated at some of the top nursing schools in the country. This experience and education gives the current leadership team at Footprints Case Management the knowledge and expertise to provide the best possible care for their patients.
