Foundation for Healthy Communities
Dakota Hayes is an experienced planner with a strong background in emergency management and public assistance programs. Currently serving as a Planner for the Granite State Health Care Coalition at the Foundation for Healthy Communities since August 2022, Dakota previously held roles at the New Hampshire Department of Safety, where responsibilities included coordinating updates to emergency operations plans and managing COVID-19 grant activities. Dakota's prior experience also includes positions with the National Association for State Community Services Programs and the U.S. Department of Homeland Security, where skills in project management and research were honed. Dakota earned a Bachelor of Arts in International Studies from American University, focusing on foreign policy and national security.
Foundation for Healthy Communities
The Foundation's mission is to improve health and health care delivery. The Foundation for Healthy Communities is a partnership involving hospitals, physicians, health plans, home care agencies and other organizations concerned about improving health in New Hampshire. The Foundation's primary objectives are: 1. To collect, analyze, and evaluate data about health and about the delivery, quality, management and organization of health services; 2. To promote, sponsor and conduct applied research and scientific investigation relative to quality, health delivery process and improvement and health policy; and 3. To communicate information, sponsor education and training, and facilitate innovation and access for the improvement of health and the creation of healthy communities.
