Gifford
Walter Ziske is a skilled healthcare professional with extensive experience in care coordination and health coaching. Currently serving as a Community Health Team Certified Health and Well-being Coach and Care Coordinator at Gifford Medical Center since May 2021, Walter Ziske focuses on assisting medically complex and high-risk patients in improving their health and quality of life. Additionally, Walter Ziske has been a Lifestyle Coach for the CDC T2 Diabetes Prevention Program with Vermont Blueprint since March 2018, raising awareness about prediabetes among the adult population. Previous roles include Population Health Coordinator and Panel Coordinator at The University of Vermont Health Network - Central Vermont Medical Center, and Senior Clinical Secretary at Dartmouth-Hitchcock. Walter Ziske holds multiple degrees from Franklin Pierce University, including Bachelor's degrees in Healthcare Administration and Human Services, as well as an Associate's degree in Business Management, complemented by certification as a Health and Wellness Coach from Wellcoaches.
