Hampton Roads Community Health Center (HRCHC), formerly known as Portsmouth Community Health Center, Inc., opened its doors as a federally qualified health center on May 14, 1995, in Portsmouth, Virginia, with two doctors, one family nurse practitioner and eleven clinical support and administrative staff.

Clinical services include but are not limited to the following:

- Treatment for both acute and chronic illnesses such as diabetes, stroke, hypertension, diet and nutrition

- Women’s health services

- Family planning services

- Prenatal care and postpartum visits

- Pediatric well baby, immunizations and school and sports physicals

- Preventive dental and restorative care for adults and children

- Medication Assistance Program for prescription drugs

- On site laboratory testing

The Outreach and Enrollment team at Hampton Roads Community Health Center is dedicated to increasing community awareness of available healthcare services and programs. They engage with local populations to provide information on health insurance options, assist individuals in enrolling in health coverage, and facilitate access to medical and preventive care resources, ensuring that underserved communities receive the support they need for their health and well-being.
