Toby Smith, DO, CMD has a diverse and extensive work experience in the medical field. Toby currently holds the role of Medical Director - Supportive Care at HMSA since March 2020. Prior to this, they served as the Medical Director at Hospice of Kona Inc. starting from February 2019. Additionally, they owned a private practice from February 2019. Previously, they were the Vice President and Chief Medical Officer at SilverSage Physician Services and also held the position of Medical Director at The Villas and Ann Pearl Rehab and Healthcare from September 2017 to August 2020. Before that, they worked as a Nursing Home Physician at Life Care Center of Kona from June 2013 to September 2017, and at Life Care Physician Services from October 2011 to June 2013.

Toby Smith, DO, CMD completed a residency program in Geriatric Medicine at the University of Hawaii John A. Burns School of Medicine from 2008 to 2009 and a residency program in Internal Medicine from 2005 to 2008 at the same institution. Toby obtained a Doctor of Osteopathic Medicine (D.O.) degree from Western University of Health Sciences in 2005. Prior to that, they earned a Master of Science (M.S.) degree in Kinesiology and Exercise Science from California State University, Northridge, from 1998 to 2000.
