Sarah Jemley

Sarah Jemley, RN, MSN, ACM, CMAC has a diverse work experience spanning over 14 years. Sarah currently holds the position of Director of Care Support at HealthPoint (CHC) since January 2021. Prior to that, they worked as the Manager - Office of Care Management at Dartmouth-Hitchcock from August 2018 to January 2021. Sarah also served as the Care Coordination Program Administrator at OneCare Vermont from January 2017 to August 2018. Sarah's experience includes managerial roles such as Manager Care Management at Northwestern Medical Center from December 2014 to April 2017 and Assistant Director of Nursing at HCR ManorCare for a brief period in 2014. Sarah began their career as an RN at Comprehensive Mental Health, followed by roles at Community Health Care and CHI Franciscan Health. Sarah has specialized in various areas including transitional care management, acute inpatient care, and clinic nursing. Sarah'sextensive experience demonstrates their expertise in care coordination and management.

Sarah Jemley, RN, MSN, ACM, CMAC, earned their Bachelor of Arts (B.A.) in History from the University of Washington in 1999-2003. Sarah later pursued a Master of Science in Nursing - Care and Outcomes Management from Pacific Lutheran University, which they completed between 2007 and 2010. Sarah also holds additional certifications, including the Case Management Administrator Certification (CMAC) and the Accredited Case Manager (ACM), obtained in 2021 and 2015, respectively. Sarah is also a Registered Nurse, certified by the Department of Health (WA Health) in 2008.
