The Clinical Outreach team at Hearth Hospice focuses on building and nurturing relationships with healthcare providers, community organizations, and families to ensure seamless access to hospice care. This team, comprised of clinical liaisons, case management nurses, and social workers, educates stakeholders about hospice services, addresses patient needs, evaluates eligibility, and facilitates coordinated care transitions, all while prioritizing compassion and support for both patients and their families.
