Hospice Care Team

The Hospice Care Team at Hospice of New York is a multidisciplinary group dedicated to providing comprehensive, compassionate care to patients with life-limiting illnesses. The team includes a Hospice Social Worker, who supports the emotional and social needs of patients and their families; a Hospice Clinical Liaison, who coordinates patient care and communication between healthcare providers; a Community Hospice RN, who delivers direct medical and nursing care; a Music Therapist, who uses music to enhance emotional well-being; and a Bereavement Coordinator, who offers grief support to families before and after the loss of a loved one. Together, they ensure a holistic approach to end-of-life care that meets the spiritual, emotional, physical, and psychosocial needs of patients and their families.
