Jane Burns

Jane Burns is a healthcare professional with extensive experience in social work and care management. Currently serving as a Referral Liaison at Hospice of the Upstate since December 2022, Jane previously worked at Cascades Verdae/SLC and Wellmore of Tega Cay/SLC as a Lifestyle Advisor, focusing on sales and marketing for skilled nursing services. Jane's experience also includes working as a Humana Care Manager, where home visits were made to ensure seniors received necessary post-hospitalization services. With a solid background in social work from Hospice of the Upstate from 2001 to 2015, Jane provided crucial psychosocial support and counseling to patients and families.
