Ephraim Back is the current Chief of Clinical Initiatives at Hudson Headwaters. Ephraim has also served as the Program Director at Mid-Hudson Family Medicine Residency Program/ Institute for Family Health, and the Associate Director at Ellis Medicine Family Medicine Residency Program. Dr. Back has also been a Medical Director at Schenectady Family Health Services (FQHC) and a Physician at the Indian Health Service. Ephraim has also served as an EIS (Epidemic Intelligence Services) Officer at the Centers for Disease Control, and a Medical Officer at the Commissioned Corps of the U.S. Public Health Service.
Ephraim Back's educational career includes a Bachelor of Science from Cornell University, a Doctor of Medicine from the University at Buffalo, and a Master of Public Health from the University at Albany School of Public Health. Ephraim also completed a Family Medicine Residency at the University of Rochester/Highland Hospital and is certified by the American Board of Family Medicine, the American Academy of Family Physicians, and the American Board of Preventive Medicine in Addiction Medicine.
Ephraim Back reports to D. Tucker Slingerland, CEO. They work with Colleen Quinn - Chief of Medical Education & Wellness, Patti Hammond - EVP & COO, and Linda Spokane - VP, Population Health Management.

Hudson Headwaters Health Network is a not-for-profit, community-based organization serving nearly 100,000 North Country residents from 21 health centers between South Glens Falls and the Canadian border. The network delivers specialty services in obstetrics and gynecology, behavioral health, care management, dentistry, labs, and imaging, and collaborates with other organizations to deliver specialty care in underserved areas.
