EVP, Network Strategy at Hudson Headwaters

Tracy Mills is currently the EVP of Network Strategy at Hudson Headwaters. Prior to this, they were the Vice President of Planning at Glens Falls Hospital from June 2012 to December 2021. Before that, Tracy was a Clinical Account Manager at Capital District Physicians' Health Plan (CDPHP) from May 2011 to June 2012. In this role, Tracy managed consultative relationships with over 20 Shared Health employer groups and their brokers. Tracy also analyzed group medical claims and other employer-specific data to identify health factors that are expected to drive future costs and utilization.

In their previous role as Coordinator of Program Development for the NYS Department of Health's Diabetes Prevention and Control Program, Tracy utilized current prevention and management guidelines to develop strategic and data-driven recommendations for health promotion and wellness initiatives. Tracy also met quarterly with employer group leadership to review data and recommend strategies to improve overall healthcare trends. Additionally, Tracy coordinated enhanced delivery of CDPHP health management services to group members and dependents, including case management, disease management, quality improvement, medication therapy management, behavioral health support and health promotion and wellness programming.

Tracy Mills has a MPP from William & Mary, a BA from Colgate University, and a certification from The Chapman Institute in Certified Wellness Program Coordinator. Tracy is currently pursuing a degree in public policy at Stockholm University.

Tracy Mills reports to D. Tucker Slingerland, CEO. They are on a team with Colleen Quinn - Chief of Medical Education & Wellness, Suzanne Blood - Chief of Palliative Care & Geriatrics, and William Borgos - Chief Medical Officer.
