Sandra Martinez-Reyes

Sandra Martinez-Reyes is a dedicated social worker with a rich background in care management and social services. Currently serving as a Social Worker Care Manager at Independence Care System since April 2016, Sandra collaborates within an interdisciplinary team to develop comprehensive care plans for a caseload of up to 50 members, addressing a wide range of needs including medical, behavioral health, and financial issues. Prior experience includes roles as a School Based Social Worker at Morris Heights Health Center from February 2012 to April 2016, where Sandra provided assessments and treatments to students, and as a Case Manager/Counselor at Harlem United from January 2011 to January 2012, focusing on individuals impacted by HIV and mental illness. Sandra holds a Master’s degree in Social Work and a Bachelor’s degree in English Language and Literature from Lehman College.
