Hazel Church

Hazel Church has a strong background in healthcare management, with a focus on clinical operations and nursing. Hazel currently holds the position of Senior Director of Clinical Operations at Keck Medicine of USC since October 2019. Prior to this, they served as the Regional Operations Director at Valor Healthcare from September 2012 to September 2019. Hazel also worked at Novia CareClinics, where they held multiple roles including Director of Nursing & Clinic Operations from April 2011 to August 2012, and Director of Nursing from July 2010 to August 2012. Hazel started their career at Valor Healthcare Inc. in 2005 as VP of Training, a position they held until 2010.

Hazel Church has a Bachelor's degree in Healthcare Administration from Bellevue University, which they obtained in 2007. Hazel further pursued their education at the same university and earned a Master's degree in Healthcare Administration in 2011. Additionally, Hazel holds an Associate Degree in Registered Nursing from Indiana State University. In 2019, they acquired the RN certification from Nevada and became a Certified Professional in Healthcare Quality™ (CPHQ) through the National Association for Healthcare Quality (NAHQ).
