Connie Soares, BSN, RN, possesses extensive nursing experience across various healthcare roles. Currently serving as a Registered Nurse at HealthCare Support since July 2020 and at Matrix Medical Network since December 2017, Connie has contributed to COVID testing efforts. Additional experience includes positions as an RN Assessor at LTCG and a Nurse Assessor at LifePlans, Inc., focusing on care coordination assessments for long-term care insurance. As an Independent FieldScout Provider with CareScout, Connie conducts assessments and facility evaluations. Prior roles include providing medical care for the ICE Health Service Corps and coordinating clinical research at Premier Research. A registered nurse since 2012, Connie also has experience in hospice and behavioral health nursing. Connie earned a Bachelor of Science in Nursing from the University of Texas at Tyler in 2011.
