Alexander Horak

Home Health Clinical Leader at Meadowlark

Alexander Horak, RN, is the Home Health Clinical Leader for Meadowlark Home Health. His responsibilities include ensuring the quality and safe delivery of home health care services; coordinate services to reflect Meadowlark’s standards of care; plans, develops, implements and evaluates home health services, programs and activities.

Prior to joining the Meadowlark team as Home Health RN Case Manager in February 2016, Alex worked as a paramedic for Via Christi Hospital in Manhattan for five years. Alex has also held positions with Pottawatomie County EMS and Saint Francis Hospital Topeka.

Alex graduated with an associate degree in nursing and an RN license from Hutchinson Community College in 2015. He graduated from the College of Emergency Services in 2010 with an associate of applied science in emergency medical technology and a paramedic license.

Originally from Maple Hill, Kan., Alex now lives in Wamego with his wife, Margaret, and their three children – Zachary, Jacob and Bethany.
