Makoto Hang

Makoto Hang is a Clinical Staff Pharmacist at Minnesota Community Care since July 2021, focusing on chronic disease management through collaborative practice agreements. Prior experience includes roles as an MTM Pharmacist at UCare, a Staff Pharmacist at CUB Pharmacy Supervalu Pharmacies Inc., and a Pharmacy Intern at Phalen Family Pharmacy, North Memorial Health, and Healtheast St. Johns Hospital. Responsibilities have ranged from medication reviews and patient education to medication reconciliation and preparing medications in compliance with safety standards. Makoto Hang obtained a Doctor of Pharmacy (PharmD) from the University of Minnesota, along with a Bachelor's Degree in Biology and an Associate of Arts degree.
