You will help clients reintegrate into their daily lives by offering therapeutic follow-up, mentorship, and ensuring a high standard of service. You'll manage client scheduling, facilitate program activities, and provide individual case management to support recovery in sober living. To provide the necessary guidance, support, and therapeutic follow up that will enable each client to effectively utilize tools and resources throughout their time in Extended Care and as they reintegrate into their daily lives

Schedule:

Monday through Friday, 8:30 am - 5:00 pm

Your Role:

  • Provide follow-up and support to clients in Extended Care.
  • Communicate effectively and timely with designated clients, families and colleagues.
  • Address non-therapeutic needs of clients while maintaining a professional demeanor.
  • Prepare for new admissions, ensuring all materials are in place.
  • Complete the admissions process, including greeting new clients and their families.
  • Orient new clients to the facility, programs, and services.
  • Communicate behavioral problems or safety concerns to Managers and Counselors.
  • Perform routine inspections of client rooms, reporting infractions of resident guidelines.
  • Facilitate program groups and other activities as needed.
  • Aid team members to ensure clients receive an unsurpassed level of service.
  • Manage scheduling for extended care clients.
  • Provide real-time mentorship and guidance to residents.
  • Assist with arranging transportation for residents to meetings and appointments.
  • Ensure residents attend the required number of recovery support meetings.
  • Contact clients' emergency contacts, family members, and former providers.
  • Formulate and implement recovery plans programs for residents.
  • Coach clients individually and in groups to overcome alcohol and drug dependency.
  • Provide weekly updates to primary contacts and referents.
  • Refer residents to other support services as needed.
  • Monitor clients' progress and adapt treatment plans as necessary.
  • Assist with coordinating treatment for clients with Outpatient Services.
  • Prepare and maintain reports and case histories.
  • Ensure proper documentation in EMR

Qualifications:

  • High School Graduate or equivalent in GED
  • Current or working towards certification (CAC, CASAC, CPRS)
  • Minimum 1 years’ experience/knowledge in substance abuse treatment preferred
  • Clean Driving Record 

Compensation:

The base rate of pay for this position is $16.00 to 24.00 per hour. Actual pay is determined based on a number of job-related factors including skills, education, training, credentials, experience, scope and complexity of role responsibilities, geographic location, performance, and working conditions.
