Michelle Wilcox, MPH, HPM is an experienced Site Manager at Neighborhood Family Practice's Puritas Community Health Center, providing administrative leadership for healthcare services. Michelle also serves as a Community Engagement & DEIB Co-Chair, implementing initiatives to improve patient outcomes. With a background in community outreach, program management, and nursing assistance, Michelle has demonstrated expertise in improving health equity and wellness initiatives.
Neighborhood Family Practice
Neighborhood Family Practice was founded in 1980 in response to a concern that residents of Cleveland’s near west side lacked access to primary health care. A Federally Qualified Health Center (FQHC), NFP focuses on providing high quality primary care in a medical home setting. As a health care provider, our seven convenient locations offer same day appointments. The NFP service area spans twelve neighborhoods across Cleveland and Lakewood and is comprised of a very diverse mix of residents. NFP accepts Medicaid, Medicare, most commercial insurance plans and offers a financial assistance program (sliding fee scale) for the uninsured. Neighborhood Family Practice is a grantee of the Health Resources and Services Administration (HRSA) and each of its locations are covered by the Federal Tort Claims Act (FTCA).
