Pamela Hammon Lcsw

Pamela Hammon, LCSW, has extensive experience in social work and behavioral health, currently serving as a Behavioral Health Clinician at One Community Health Sacramento since September 2021 and as a Social Worker at Range and Hammon, LLC since 2014. Previous roles include working with oncology and nephrology clients at the U.S. Department of Veterans Affairs and serving as a Mental Health Case Manager at Sierra Vista Hospital. Pamela has also transitioned clients to independent living as an MSW, ASW Case Manager CCT Coordinator at Choice in Aging and supported individuals with disabilities in a state program while working as an MSW Case Manager at Home and Health Care Management. Additional experience includes serving as a Medical Social Worker at VITAS Healthcare. Pamela holds a Master's degree in Social Work and a Bachelor's degree in Social Work from Brescia University.
