The Healthcare and Community Engagement team at the Patient Advocate Foundation works to improve health equity by connecting patients with vital resources, providing personalized case management services, and offering educational programs to empower individuals facing chronic, life-threatening and debilitating illnesses. This team collaborates nationally to ensure comprehensive support and advocates for underrepresented communities to enhance their access to healthcare.
