Heather Witt

Heather Witt, MOL, LSSGB, is a seasoned professional with extensive experience in healthcare and social services. Currently serving as a Revelation Wellness ® Instructor for an independent entity since April 2021, Heather has a strong background in management and quality improvement. At Phelps Health, Heather holds multiple roles, including Clinic Manager, Human Resources Business Partner, Lean and Quality Improvement Coordinator, and Lean/QI Specialist, where responsibilities include leading performance improvement initiatives and coaching organizational members. Prior experience includes serving as a Social Worker for RehabCare and Kindred Hospital Rehabilitation Services, and as a Social Service Worker for the State of Missouri, focusing on child welfare and family support. A former QI Analyst and Care Manager at Phelps County Regional Medical Center, Heather has consistently contributed to the analysis and enhancement of quality improvement activities.
