Jonathan Leizman

Jonathan Leizman, M.D., currently serves as the Chief Medical Officer at Premise Health since April 2019. Previously, Jonathan held the position of Medical Director at Cleveland Clinic At Work from July 2013 to March 2019, where responsibilities included developing, building, operating, and managing on-site employee health clinics. Jonathan earned a Doctor of Medicine (M.D.) from The Ohio State University College of Medicine (1993-1997) and completed a Family Medicine Residency Program at University Hospitals of Cleveland (1997-2000). Jonathan holds a Bachelor of Arts in English Language and Literature from Dartmouth College (1989-1993) and is a graduate of Cleveland Heights High School (1985-1989).
