Theresa A. Soriano

Theresa A. Soriano is the Associate Chief Clinical Officer at Prospero Health. Theresa has held this position for 1 year and 5 months. Most recently, Theresa served as Senior Vice President, Care Transitions and Population Health at Mount Sinai Morningside for 2 years and 9 months before taking their current role at Prospero Health.

Throughout their career, Theresa has been dedicated to improving access to healthcare for underserved populations. Theresa has served as Deputy CMO Population Health and Medical Director Mount Sinai Health Home at the Mount Sinai Health System and Director of the Mount Sinai Visiting Doctors Program at The Mount Sinai Hospital. In these roles, they have worked to improve care coordination and transitions between different levels of care, as well as develop innovative models of care delivery that focus on population health management.

Theresa A. Soriano has an MD and MPH from the Icahn School of Medicine at Mount Sinai, a BS from Cornell University in Human Development, Family Studies, and Related Services, and an Internal Medicine Residency from the University of Miami, Miller School of Medicine.
