Client and Community Support

The Client and Community Support team at REAL Services is dedicated to addressing the diverse needs of elderly, disabled, and low-income individuals in Northern Indiana. This team provides comprehensive assistance including care management, service coordination, health support, senior and client support, asset building, and program management. They work collaboratively with hospitals, community services, and area agencies on aging to ensure clients receive the necessary resources, education, and protection to improve their quality of life and achieve self-sufficiency.
