Here's what you'll be doing

We’re looking for an experienced Diabetes Care Specialist to join our team. You’ll be responsible for patient experience and success of our practice, including: 

  • Counsel and educate patients and families appropriately on diabetes care in collaboration with the care team 
  • Complete nutritional assessments and make appropriate recommendations as a part of the care planning process 
  • Analyze CGM data and translate to treatment changes for patients 
  • Communicate with patients through video calls, in person meetings, telephone and electronic messaging 
  • Create and maintain documentation 
  • Ensure patient satisfaction by building rapport and maintaining positive relationships with patients and their families 
  • Other duties as assigned 

What you need to be successful

  • Registered Nurse (RN) or Registered Dietitian (RD) and Certified Diabetes Care and Education Specialist (CDCES) required 
  • Licensed or registered in the state of practice as required by state regulations 
  • Prior experience leveraging CGM data 
  • Passion for the impact of your work on the broader world, and excitement to help reduce the burden of chronic disease
