Nicole Bloomfield, MSSW, serves as a Senior Case Manager at Seven Counties Services, focusing on the Home & Community Based Waiver since February 2022. Previously, Nicole held the position of Principal Case Manager at Central State Hospital, coordinating treatment plans and facilitating access to community services for patients. Nicole also worked as a Senior Case Manager for "Project Link" at Seven Counties Services: Addiction and Recovery, providing specialized support for pregnant and postpartum women with substance use disorders. Earlier roles include Senior Case Manager at Centerstone Kentucky and Community Living Supporter at Employment Solutions, Inc., promoting independence and life skills among individuals with disabilities. Nicole holds a Master's degree in Clinical/Medical Social Work from the University of Louisville and a Bachelor's degree in Kinesiology and Health Promotion from the University of Kentucky.
