What Are We Looking For We are looking for like minded Primary Care Physicians to join us on our journey. This individual would be responsible for managing patient care and treatment in collaboration with other team members.

Responsibilities

  • Provide exceptional patient care
  • Perform health assessments and screen for preventative health measures
  • Active medical management of common and complex conditions
  • Order, interpret and evaluate diagnostic tests to identify and assess patients’ clinical problems and health care needs
  • Formulate and document care plans
  • Explain medical procedures and plans of care to patients and their families
  • Prescribe medication or other forms of treatment.
  • Participate in an on-call rotation, as needed
  • Communicate assessment findings and collaborate in identifying gaps in care

Qualifications

  • Board eligible or board certification in Internal Medicine, Family Medicine, or Geriatrics.
  • Current CT or AZ license (or eligibility to apply)
  • DEA license (or eligibility for registration)
  • Demonstrated interest/passion for value-based care delivery models
  • Strong oral and written communication skills
