Lydia Mayfield, BSN, RN, is a highly experienced nursing professional with a diverse background primarily in addiction services, behavioral health, and forensic nursing. Currently serving as a MAT Nurse Care Manager at Signature Health, Inc. since July 2019, Lydia provides comprehensive nursing services to opioid-dependent patients. Previous experience includes working as a SANE Nurse at University Hospitals, where forensic exams and evidence collection were performed, and as a Registered Nurse with Matrix Medical Network focusing on clinical trials. Lydia's nursing career began at Cleveland Clinic, where significant roles included inpatient behavioral health and geriatric medicine. Lydia holds a BSN from Ohio University and various nursing and health-related qualifications, underpinned by a Bachelor of Arts in Social Work from Cleveland State University.

AGENCY OVERVIEW Founded in 1993, Signature Health is a Federally Qualified Health Center (FQHC) providing mental health, addiction recovery, primary care, infectious disease, pharmacy and lab services to patients of all ages. With 10 outpatient and residential locations in three Northeast Ohio counties (Ashtabula, Cuyahoga and Lake), Signature Health primarily serves Medicaid and Medicare patients and offers a sliding fee scale based on household income for patients without insurance.
