Mordy Eisenberg

Mordy Eisenberg, NHA, is a skilled clinician, administrator, entrepreneur and solution finder. He has held various roles in health care since 1994. Mordy was most recently the chief of strategy and value-based care at Centers Health Care (CHC), the largest and most complete post-acute continuum in the Northeast. Before assuming this role, Mordy was director of clinical operations and was responsible for the development and oversight of the corporate clinical teams. The CHC continuum encompasses skilled nursing facilities, assisted living programs, adult day health care, homecare, durable medical equipment, ambulance, managed care products and other ancillary companies. Mordy’s efforts at bringing the continuum to life have been exceptional, with his primary focus on clinical quality improvement, value-based care initiatives and integrating the continuum.
