David S. Forman

CAPT Forman, a native of Southern California, graduated with merit from the U.S. Naval Academy in 1998 with a Bachelor of Science degree in History. He became Airborne qualified as a midshipman and SCUBA qualified before starting the Nuclear Power training pipeline. CAPT Forman has been class president since his graduation.

As a submarine warfare officer, CAPT Forman served on five submarines that collectively completed four fast-attack deployments and seven strategic deterrent patrols.

CAPT Forman was the Commanding Officer of USS ALASKA (SSBN-732) (BLUE) from March 2015 to September 2017. ALASKA earned the 2016 Squadron TWENTY Battle Efficiency “E” and the 2016 Commander, U.S. Strategic Command OMAHA Trophy as the best fleet ballistic missile submarine. CAPT Forman also earned the 2017 Naval Submarine League Rear Admiral Jack N. Darby Award for Inspirational Leadership and Excellence in Command.

Ashore, CAPT Forman earned a 2005 Masters degree in Operations Analysis from the Naval Postgraduate school and a 2014 National Security Strategy degree from the National War College, where he is also a distinguished graduate. From 2008 to 2010 he was the Flag Secretary to the Naval Academy Superintendent, and in 2013 he served as the Navy Fellow at the Washington D.C. think tank, the Center for a New American Security (CNAS).

Prior to assuming the duties as the Deputy Commandant of Midshipmen, CAPT Forman served four years in the Pentagon. First, he was the Deputy Director for Navy Strategic Systems in the Office of the Secretary of Defense (Acquisition and Sustainment), and second, as the Assistant for Sea-Based Strategic Deterrence to the Director, Undersea Warfare (OPNAV N97). Both positions involved budgeting and acquisition oversight of the Navy’s #1 priority of strategic deterrence.

His personal awards include the Legion of Merit, Defense Meritorious Service Medal, Meritorious Service Medal (two awards), Navy and Marine Corps Commendation Medal (three awards), the Navy and Marine Corps Achievement Medal (two awards), and numerous campaign and service awards.
