Jacqueline Brysacz is a highly skilled Family Nurse Practitioner with extensive experience in providing comprehensive bilingual family medicine across diverse populations. Currently serving at Union Community Care since January 2018, Jacqueline delivers full-scope care, including prenatal and newborn services, to patients regardless of their insurance status. Jacqueline also holds the position of Chairman Of The Executive Board at the Chaco Foundation, focusing on empowering young scholars in Bolivia. Previous roles include serving as a Family Nurse Practitioner at Hamilton Health Center, where care was provided to a high-need community, and as a Family Nurse Practitioner Resident at Community Health Centers, Inc. With a strong educational background from The Johns Hopkins University in Family Nurse Practitioner and Public Health, alongside earlier positions in public health and infectious disease management, Jacqueline demonstrates a dedicated commitment to improving healthcare access and outcomes for underserved populations.

Now, we set out on the long journey of supporting patients recovering from COVID-19, vaccinating the hard-to-reach and vulnerable among us, and helping families and communities heal emotionally and physically. We understand and embrace the complex lives and unique strengths of our patients, and we work hard to break down all barriers to care. This means we look through a grassroots lens. We go right into our community because we are our community. We are all a neighbor, a friend, a family member, and collectively, a trusted health home.
