Diane Silbernagel

Since 1981, Diane Silbernagel has worked in community-based care as a Visiting Nurse and in roles involving management and administration of community-based services for populations across the lifespan. In 2016, Diane received a Doctorate of Nursing Practice from St. Peter’s University and joined Home Care Options as the Executive Director after having served on the Agency’s Board of Trustees since 2002. Diane has served in leadership roles for various organizations including Catholic Charities USA, Newark Planning Council for Ryan White Services, and the Northern NJ Parish Nurse Network. In 1996 she participated in the CMS National Medicare Demonstration Project, MEQA, which resulted in the implementation of OASIS data collection and the transition to Prospective Payment/HHRGs in Home Health Care. Formerly a CPHQ, Diane is a trustee at the Commission on Accreditation for Home Care. She also serves as a trustee for the Passaic County, Care Management Organization, Circle of Care.
