Amy Perkins

Amy Perkins has over 20 years of experience in social work and case management within the healthcare sector. Currently, Amy serves as a Case Manager at Western Reserve Area Agency on Aging since February 2020. Prior experience includes positions as a Care Manager at Professional Edge Nursing, Inc., and various social work roles at Heartland Home Care & Hospice, where responsibilities involved supporting home care patients and facilitating transitions to hospice services. Amy also held leadership positions in social work at Sylvania Nursing Home and assisted patients and families with social needs in earlier roles at Villa North Nursing Home and Beach Nursing Home. Amy's career began as a Discharge Planner at ProMedica-Flower and Toledo Hospitals. Amy holds a Bachelor's degree in Social Work from the University of Toledo.
