Rosa Tabi Lmsw

Rosa Tabi, LMSW, has extensive experience in social work, currently serving as a Community Support Specialist at AiRCare Health since January 2022. Prior roles include Clinical Social Worker at Jackson Memorial Hospital, where responsibilities encompassed assessments, case management, and collaboration with a multi-disciplinary team for patient discharge planning. At Mount Sinai Health System, Rosa conducted comprehensive bio-psychosocial assessments and provided crisis intervention for patients facing chronic illnesses. Additional experience includes roles as an EM Case Manager for the Rocky Mountain Offender Management System and a Case Manager Aide at EAC Network. Rosa holds a Master of Social Work from Fordham University and a Bachelor of Arts in Sociology from St. Francis College, and is currently a doctoral student in Social Work at Capella University.
