Health Care Management Team

The Health Care Management Team at Alliance Health is dedicated to coordinating and optimizing the delivery of mental health, intellectual and developmental disability, and substance abuse services across the counties served. This interdisciplinary team focuses on enhancing community care through effective case management, education, and outreach initiatives, ensuring that Medicaid-eligible individuals receive comprehensive, whole-person care.
