Anne Swido

Anne Swido is a highly experienced Nurse Practitioner with a robust background in case management and urgent care across multiple healthcare settings. Since September 2016, Anne has been serving as the sole/lead NP at Community HousingWorks, focusing on managing complex geriatric medical and psychiatric conditions to reduce unnecessary emergency room and hospital admissions. Previous roles include significant positions at Long Island College Hospital, where Anne provided care in a high-volume ER, and at the Hospital for Special Surgery, specializing in post-surgical patient management. Anne holds a Post-master's Certificate in Family Nurse Practitioner from Pace University and a Master of Science in Nursing, as well as a Bachelor of Science in Nursing from Columbia University.
