Tracy Telander, RN BSN PHN CCM MBA, has a diverse work experience in the healthcare industry. Tracy began their career at Gillette Children's Specialty Healthcare as a Registered Nurse from May 2003 to May 2006. Tracy then joined Fairview Health Services in 2006, where they worked as an RN in Homecare from May 2006 to July 2008. Tracy subsequently held a managerial role as a Nursing Manager in Care Coordination from July 2008 to April 2015 at Fairview Health Services. Tracy then transitioned to HealthEast in 2015 as a Director of Managed Care and Ambulatory Case Management, a position they held until May 2018. Tracy returned to Fairview Health Services in 2018 and has since held various roles, including System Director of Ambulatory Care Management and System Staffing and Scheduling from May 2018 to June 2020, System Director of Connected Care from June 2020 to November 2022, and currently serves as the Vice President of Clinical Integration. Tracy's extensive experience in both clinical and managerial roles makes their a valuable asset to the healthcare industry.

Tracy has also acquired additional certifications throughout their career, including Public Health Nursing Certification from the MN Board of Nursing in September 2019, Certification in Case Management from the Commission for Case Manager Certification (CCMC) in May 2013, and Registered Nurse certification from the MN Board of Nursing in July 2003.
