Kristin Hoats

Kristin Hoats, MSW, LSW, currently serves as a Clinician at Forge Health, where responsibilities include facilitating intensive outpatient programs and assessing clients for substance use disorders and mental health needs. Previously, Kristin held the position of Wellness Coordinator at Stop Soldier Suicide, conducting clinical risk assessments and establishing long-term relationships with at-risk clients. Experience also includes working as a Case Manager for Advanced Illness at Geisinger, assisting chronically ill patients in person-centered decision making, and serving as a Medical Social Worker II at Fresenius Medical Care North America, where assessments and counseling were provided. Kristin's professional journey began with a Virtual Field Practicum at the University of Southern California, where live therapy practices were conducted. Kristin earned a Master’s Degree in Social Work from the University of Southern California and a Bachelor’s Degree in Professional Studies with a concentration in Social Work from Misericordia University.
