Tammy Polk

Tammy Polk is the current Chief Human Resources Officer at Formstack. Tammy has also served as the Chief People Officer at Four Winds Interactive from February 2021 to March 2022, and as the Vice President Global People Partnerships at Lightspeed HQ from December 2020 to February 2021. Prior to that, they were the Executive Vice President, Talent at Upserve from October 2018 to December 2020.

Tammy has a long history of HR experience, and has been instrumental in transforming businesses and overseeing global HR operations. Tammy has created performance processes, improved employee engagement, and restructured organizations to position them for growth. In their most recent role at Formstack, they are responsible for leading the company's HR strategy and initiatives.

Tammy Polk has a BS in Resource Management from Iowa State University. Tammy is also certified as an Insights in Discovery Practitioner.

Tammy Polk reports to Chris Byers, CEO. They are on a team with Rob Lawrence - Chief Operating Officer, Richard Yu - Chief Product Officer, and Liza Smyth - SVP, Customer Experience.
