Denise Patriaco

Denise Patriaco is an accomplished healthcare professional with extensive experience in population health and clinical affairs. Currently serving as Regional Director of Population Health at Hackensack Meridian Health since October 2016, Denise also holds the position of Vice President of Clinical Affairs at Regional Cancer Care Associates, where efforts focus on providing high-quality, compassionate cancer treatment across New Jersey, Maryland, and Washington D.C. Previous roles include Vice President of Care Management at Remedy Partners, Director for Care Coordination at Hackensack Alliance ACO, and APN and Manager of Clinical Performance Outcome Measures at Hackensack U Medical Center. Denise holds a Master of Nursing in Family Practice from Pace University and is a Doctor of Nursing Practice candidate at Duke University.
