Heart of Ohio Family Health is the parent company for five Federally Qualified Health Centers  located in Franklin County:  Capital Park, Chantry, Mount Carmel East, Whitehall, and Whitehall-Yearling High School.  Our mission is to provide high-quality, holistic, and compassionate care to meet the healthcare needs of everyone in our diverse community, one heart at a time. We provide services regardless of ability to pay, and offer a sliding fee scale based on income and number of dependents.

The Healthcare and Clinical Services team at Heart of Ohio Family Health is dedicated to providing comprehensive, patient-centered medical care. This multidisciplinary group includes roles such as RN Clinical Care Coordinators, HIV Case Managers, and Family Physicians, who collaborate to deliver holistic health services. They address a wide range of needs from chronic disease management and psychiatric care to primary and preventive services, ensuring each patient receives compassionate and individualized attention, regardless of their financial situation.
