About the Role

 As an Outcomes Lead in Home and Living, you will play a pivotal role in mentoring and guiding a team of Disability Support Workers to deliver high-quality, person-centered outcomes for the people we support. Your responsibilities include: 

  • Understanding, developing, and refining individual programs of support in line with a person’s NDIS goals
  • Developing links to a person’s community that offer them greater opportunity for inclusion and the development of valued social roles
  • Onboarding, supporting, supervising, and developing of assigned team members
  • Managing transport logistics during service delivery
  • Collaborating on service quality monitoring, planning, analysis, and reporting
  • Working with the Leadership team to ensure smooth rostering and system updates
  • Collaborating with peers to maintain accurate person-supported information and documentation
  • Reviewing and approving Carelink entries in line with payroll and billing requirements
  • Delivering hands-on disability support work as required
  • Student placement supervision as required

You will:

  • Be a natural influencer, modelling a positive attitude, professional conduct and unwavering commitment to quality outcomes and service delivery for staff, individuals with disability and the community.
  • Possess a deep understanding of the NDIS, its funding, and systems.
  • Have experience in leading and inspiring big teams
  • Have prior support worker experience including demonstrated understanding and knowledge of the tasks of a disability support worker
  • Be committed to service excellence, human rights, and zero tolerance to violence, abuse, neglect, and exploitation.
