Kate Freeman

Kate Freeman has a diverse work experience in the healthcare industry. Kate currently serves as the Director, Client Partner Implementation at Navvis, where they utilize their deep knowledge of value-based care and population health strategies to guide the implementation of solutions for clients. Prior to this, they worked at AdventHealth as the Director, Population Health Operations, where they focused on practice transformation optimization and centralized provider education on population health. Kate also held the role of Director, Provider Engagement and Performance at AdventHealth, where they provided expertise and consultation on population health best practices and built performance improvement plans for providers.

Before joining AdventHealth, Kate worked as a Senior Consultant at COPE Health Solutions, where they provided solutions to healthcare clients in various areas including clinical redesign and financial modeling. Kate also had experience at SSM Health in multiple roles, such as Manager, Population Health and Value-Based Care, Manager, Value-Based Care Delivery, Supervisor of Patient Access, Supervisor of Financial Clearance, and Patient Access Representative. In these roles, they played a key role in managing population health strategies, leading operational initiatives, and providing direction to teams.

With their extensive experience in value-based care and population health, Kate brings a wealth of knowledge and expertise to their roles in the healthcare industry.

Kate Freeman earned a Bachelor of Science degree in Biology, with a minor in Chemistry, from the University of Missouri-Saint Louis, where they studied from 2006 to 2013. Later, they obtained a Master's degree in Health/Health Care Administration/Management from Webster University, where they studied from 2013 to 2014.
