Susan Hooper

Susan Hooper, MSW, has extensive experience in social work and care coordination, currently serving as CHOICES Care Coordinator and CCP Care Coordinator at Oak Park Township since October 2021. Previous roles include Social Services Manager at Norridge Gardens, LLC, where responsibilities included overseeing unit protocols and managing the elopement program, as well as various positions within LivHOME, Lee Manor, and North Shore Senior Center, which involved care management, social services supervision, and IT support in a healthcare context. Susan holds a Master of Social Work from Dominican University and a Bachelor of Science in Social Work from Illinois State University, complemented by an Associate of Arts degree from Harper College.
