Katherine Suberlak

Senior Vice President, Population Health at Oak Street Health

Katherine Suberlak has extensive experience in the healthcare industry, specifically in population health. Katherine began their career at PCC Community Wellness Center in 2007, where they served as the Chief Population Health Officer for over 10 years. Katherine demonstrated diverse skills in clinical social work, substance use, provider relations, recruitment, project management, and grant writing. In 2018, Suberlak joined Oak Street Health as the VP of Population Health, and then went on to become the VP of Clinical Programs. Currently, they hold the position of Senior Vice President, Population Health at Oak Street Health.

Katherine Suberlak obtained a Bachelor's degree in English Language and Literature from the University of Michigan College of Literature, Science, and the Arts, where they studied from 2000 to 2004. Katherine then pursued a Master of Arts degree in Social Work at the University of Chicago from 2005 to 2007.
