The SPARK Case Manager holds partial accountability for the ongoing operations and the success of SPARK Program, which is part of the Community Action Committee of Pike County.  Reporting to the SPARK Case Manager Coordinator, the Case Manager ensures the efficiency and effectiveness of case management services provided to youth and young adults.  Through personal accountability, the Case Manager strives for maximum performance, engagement, and dedication to the SPARK Program and clients. 

Responsibilities, Deliverables, Outcomes and Expectations

·         Collects, organizes and analyzes client information that is gathered through records, tests, interviews, and professional sources.  This is used to appraise their interests, aptitudes, and personality characteristics in order to create a realistic and achievable Individual Opportunity Plan.

·         Assists individuals with completing intake and enrollment forms.

·         Adheres to established guidelines for entering data in required client tracking systems.

·         Determines eligibility and appropriateness for services according to program guidelines.

·         Ensures compliance with State and/or Federal Regulations.

·         Responsible for case management duties, reviews, follow-ups, and reports for all clients assigned.  Obtains attendance reports and reviews for potential problems.  Counsels clients as required for a successful completion of the service plan.

·         Refers customers to other agency programs as needed.

·         Provides placement in post-secondary training and/or unsubsidized employment and provides retention services to help ensure continued gainful employment.

·         Obtains attendance reports and reviews for potential problems.

·         Assists with the implementation of like skills workshops and educational workshops.

·         Responsible for placing clients on worksites and following up weekly.

·         Uses Goal4 It! Program to motivate clients to complete career and/or educational goals.

·         Assists with preparing resumes, cover letters, and practice interviews as needed.

·         Ensures compliance with State and/or Federal Regulations regarding case file management. This includes timely data entry into required database systems

·         Ensures required documentation is housed in client files.

·         Commits to procedures and policies provided by CAC, ODJFS, and Area Workforce Board regarding case file management.

·         Performs outreach and recruitment for all programs to help improve the local awareness of OhioMeansJobs Pike County and the Workforce & Business Develop Programs.

·         Communicates with area employers, students and school officials to promote the SPARK Program.

·         Workforce & Business Development Staff.

·         Perform backup duties in the OMJ Center as needed.

·         Attend meetings, conferences and training sessions related to the duties of this position and disseminate acquired information to appropriate individuals.

·         Responsible for sending information to potential clients through fliers, promotional items, discussion, etc.

·         Adheres to all agency safety policies and procedures.

·         Serves as part of a CAC committee and collaborates to promote program activities, service coordination, and continuous improvement measures.

·         Perform any other tasks as directed by Supervisor.

Minimum Requirements…

Education: 

Bachelor’s Degree in Social Work, Secondary Education, Business Administration, or Specialized Studies in degree in education or related field.  Four (4)  years’ experience in working with at-risk and/or adolescents with barriers to education/employment may be considered in lieu of degree.

Job-Related Experience:

Must complete pre-employment drug, alcohol test, Ohio BCI/FBI background check as required

Licenses, Certifications, and Accreditations:

Must have valid driver’s license and be insurable under the agency insurance.

Preferred Qualifications…

Preference given to individuals who have experience working with a diverse population in varied settings.

Preference given to individuals who have an understanding of client case management, social services, employment and training resources available within the surrounding area.

Preference given to individuals who have knowledge of client tracking systems.

Working Conditions…

Position is in a well-lighted office environment. This position requires occasional local and regional travel (up to 40%) for the purpose of engaging with assigned programs/departments. Occasional evening and weekend travel/work is required. Work is fast paced and involves multiple, sometimes competing priorities along with the management of complex project details. Can involve low-to-moderate levels of stress.
